  • All minors must have a fully comprehensive travel insurance arranged prior to the start of their course. Students are recommended to disclose any mental or physical illness, allergy. disability or condition that may interfere with their ability to successfully complete their course that may impact the wellbeing of any other student or staff member that may require monitoring, treatment or emergency intervention during the student's period of enrolment, or that may require special accommodation.

  • I/We hereby authorize my/our son/daughter to attend the course at ES ENTERPRISE SOLUTIONS DMCC and understand that the campus has a mixture of junior students and adult students from different nationalities. I/We also understand that ES ENTERPRISE SOLUTIONS DMCC is committed to safeguarding and child protection, providing supervision and support and that the program allows students to make their own arrangements during free slots in between lessons and activities.

  • I/We authorize my/our son/daughter to be housed at the Student Residence, alongside the other students within his/her group, and I understand that my/our son/daughter will share the common areas such as kitchen, laundry room, swimming pool and gym with other students residing at the student Residence.

  • Declaration

  • As the parent or legal guardian of the above-named student, I hereby give ES ENTERPRISE SOLUTIONS DMCC (and their respective agents, employees, volunteers, promoters and trustees) permission to include this student on field trips/activities/excursions accompanied by ES ENTERPRISE SOLUTIONS DMCC staff / representatives. I understand and accept that the ES ENTERPRISE SOLUTIONS DMCC staff and students will be travelling on foot or by plane, train, bus, boat, car or van to their destination.

    ES ENTERPRISE SOLUTIONS DMCC will take care of the above-named student. However, I acknowledge that on adventurous field trips/activities/excursions such as the ones ES ENTERPRISE SOLUTIONS DMCC will involve the students in, accidents do occasionally happen, and I accept that my family and I will not bring a claim against ES ENTERPRISE SOLUTIONS DMCC for death or personal injury unless ES ENTERPRISE SOLUTIONS DMCC is shown to have failed to maintain its duty of care.

    As the parent or legal guardian of the above-named student, I have procured for them such adequate insurance as may be required to provide cover for the loss and/or damage to all the possessions they take with them on ES ENTERPRISE SOLUTIONS DMCC field trips/activities/excursions, and that, in the event that they suffer loss and/or damage, I will therefore not make a claim against ES ENTERPRISE SOLUTIONS DMCC for that loss and/or damage.

    For the avoidance of doubt nothing in this waiver will limit any of the liability ES ENTERPRISE SOLUTIONS DMCC may have for death or personal injury arising as a result of ES ENTERPRISE SOLUTIONS DMCC negligence. I acknowledge that I have read this statement carefully before signing this consent form, and provide my permission to include this student on field trips/activities/excursions by signing it.

     

    To the best of my/our knowledge, my/our son/daughter is in a good state of health. Whilst all appropriate precautions will be taken for the safety of my son/daughter, any loss, damage or harm to my sons/daughters person and/or personal belongings that may arise from the program shall be exempt of all recourse.

    ES will endeavor to contact parents in case of any emergency. However, if my son/daughter require medical treatment or hospitalization during his/her stay on this program the attending physician and/or hospital is authorized to treat my/our son/daughter.

    I/We give ES ENTERPRISE SOLUTIONS DMCC the absolute right and permission to publish, copyright, distribute and use pictures and/or videos of my child (or pictures taken by my child) for any purpose which may include, among others, advertising, promotion, marketing and packaging for any ES ENTERPRISE SOLUTIONS DMCC product or service. I understand that these photos will not be used for any other than ES ENTERPRISE SOLUTIONS DMCC purposes and agree that images may be combined with other images or footage, text and graphics, and cropped, altered and modified. If my child does not wish to be part of photo or video content it will be their responsibility to absent themselves.

    I/We have read and discussed the following guidelines with my/our son/daughter and acknowledge that he/she has agreed to follow the program guidelines and further agree that the staff of ES ENTERPRISE SOLUTIONS DMCC reserve the right to expel my child from the program in the event of any guideline violation.

    I/We acknowledge that my child will participate in fire drill and safety training sessions organized by the School/Institution. I understand that these sessions are essential for educating children about emergency procedures and safety protocols in the event of a fire or other emergency situations. I agree to ensure my child's punctual attendance at all scheduled training sessions unless prevented by illness or other unavoidable circumstances.

    I/We understand that my child will be issued an access card for participation in fire drill and safety training sessions. In the event that the access card is lost or misplaced, I agree to pay a replacement fee of $100 USD to cover the administrative and material costs associated with issuing a new access card.

  • Accommodation

    • Students must keep their room neat and tidy
    • Students must not allow anyone other than those allocated to that room to sleep over
    • Students are not allowed to sleep out even in another apartment within the residence
    • Students are not allowed to invite guests into the residence
    • Students are not allowed to smoke at the residence
    • Students must not make excessive noise that could disturb others
    • Students are liable for any damages/breakages caused to property
    • Payment for damages must be effective immediately and residence staff will not allow the students to leave the residence unless payment has been made
    • Students must pay directly for any additional services not included in the program
    • Students must return their electronic access card to the reception desk when they are leaving the residence
